TO: PARENTS (LEGAL GUARDIANS) RE: FIELD TRIP PERMISSION DATE OF TRIP: Friday May 2nd to Sunday May 4th, 2008 The NDP Music Department has arranged for your daughter to participate in the “Music in the Parks” music adjudication in Williamsburg,Va. The purpose of this trip is for the students to participate in an adjudicated music festival as a member of the NDP Concert Band, Orchestra, Concert Choir and Chorale. After their performance the students will be enjoying the evening in Williamsburg Va. and the next entire day at the “Busch Gardens” theme park. The following arrangements have been approved by the school: Departure Time from NDP: May 2nd - 7:30am (The Bus will leave promptly!) Return Time to NDP: May 4th – 2:00 pm Type of Transportation: Coach Bus Cost per Student: $260.00 Supervisory Personnel: Mr. Dzbinski, Mr Kuznik, Mr. Smith, and Chaparones
Reasonable care will be taken by the supervising personnel to see to the safety of your daughter. By signing the slip below, you grant written permission for your daughter to participate in this activity. Therefore, please sign the slip below and have your daughter return it to the Music Dept. no later than: April 4th, 2008.

I, the parent/legal guardian of _______________________________________________ (Student’s name) Request that Notre Dame Preparatory School allow my daughter to participate in the Music Adjudication field trip to Williamsburg Va. on Friday May 2nd to Sunday May 4th 2008. I release and save harmless Notre Dame Preparatory School and it’s employees from all liability for any and all harm arising to my daughter as a result of this trip.
